MYTh (Marloo Youth Theatre) Program Consent Form

Congratulations! You have been accepted into the MYTh program, run by Darlington Theatre Players Inc. (DTP).

MYTh sessions will take place at either of the following locations:

Swan View Youth Centre (Brown Park)
Marloo Theatre, 20 Marloo Rd, Greenmount WA 6056

By completing this form, you confirm your commitment to the program’s rules and expectations.

Program Rules & Policies

All MYTh participants must adhere to the following:

MYTh Principles

Treat everyone with respect, including fellow participants, parents, staff, and volunteers.
Follow all instructions given by MYTh Leaders for safety and well-being.
Use appropriate language, behaviour, and dress at all times.
Report any discrimination, harassment, or bullying to a MYTh Leader.
Underage drinking, smoking, and drug use are strictly prohibited.

Costume & Theatre Etiquette

Participants are responsible for their own undergarments, hosiery/socks, and shoes.
Costumes must be stored properly when not in use.
No eating or drinking while in costume.
Respect all theatre spaces and property.

Safety Policy

Participants must remain with the group at all times.
A parent/guardian must sign out participants who leave early.
Bathroom and drink breaks require permission from a MYTh Leader.
Advance notice must be given if a participant is picked up by someone not listed as an emergency contact.

Photography, Videography & Media Release

Darlington Theatre Players Inc. (DTP) and MYTh regularly capture images and videos of participants for use in:

Promotional materials, including website, social media, and advertising
Educational and archival documentation
Newsletters and media publications

By submitting this form, you acknowledge that you may be photographed or filmed and consent to these materials being used for promotional and documentation purposes.

Program Costs & Membership

All MYTh participants must be financial members of Darlington Theatre Players Inc. before participating in MYTh activities.
